Customer Data Worksheet enables you to update your information with FSA, including your contact and demographic information. Must be updated every two years.
Highly Erodible Land Conservation and Wetland Conservation Certification. Required for FSA to have on file to be eligible for USDA programs and crop insurance. Typically, this only has to be done once.

Report of Acreage. If you do not have crop insurance, use this form to report crops planted with USDA. If you have crop insurance, file through your approved insurance provider. USDA will get your acreage information from your insurance courier.
If applicable, this certification reports income from farming, ranching, or forestry-related activities for those exceeding the adjusted gross income limitation.
*Note: Forms CCC-941 and CCC-942 (if applicable), are used for certifying average AGI and Farm AGI, are required to be filed by the applicant. The individual or a legal entity applying must provide information on all members, stockholders, or partners at or above the fourth level of ownership in the business structure.
